FORMS

To become a Landmark Nurseries, Inc. customer, complete the ‘Customer Information Form’ and provide copies of the following:

• Texas Nursery Certificate
• Texas Driver’s License
• Business Card

If you have a current Texas Sales and Use Tax number and prefer to claim an exemption from paying sales tax, the Sales and Use Tax form must be completed in its entirety and properly signed.  A sales tax exemption cannot be granted if the form is incomplete.
